AI Documentation in Hospitals: Vendors Compared (2026)

Tandem Health, Recare, voize, FICUS, aiomics: who does what, who is certified for what, and which questions separate the categories? A sober overview for hospital decision-makers.

Three vendor decks are on the table, and all three promise the same thing: less documentation burden, more time for patients, GDPR-compliant. The slides differ in their color palettes. The products behind them differ in almost everything — in what they actually process, in their regulatory status, in where the data is processed, and in the question of who ultimately checks whether the output is correct.

This overview sorts the best-known vendors for German hospitals, as soberly as possible. Statements about competitors come from their own publications and are labeled accordingly. Where we ourselves are concerned, we say so.

Four categories that tend to blur in sales decks

The market currently throws four different product categories into one pot, though they solve different problems.

  • Ambient scribes record the physician-patient conversation and turn it into notes or letters. They answer the question: what was said?
  • Extraction platforms read existing documents and transfer their contents into systems or forms. They answer the question: what do the records say?
  • Voice documentation for nursing replaces the keyboard with dictation into structured nursing records.
  • A verification layer checks extracted or generated statements against the source document and the existing record before people trust them. It answers the question: is this actually true?

Buying a scribe gives you no answer to the stack of faxes in admissions. Buying extraction gives you no answer to whether an extracted diagnosis is supported by the source document. The categories complement rather than compete with each other — which makes it all the more important to know what you are comparing.

The vendors, one by one

Tandem Health (Stockholm)

Tandem Health is Europe's reference vendor for AI writing assistance at the point of conversation. According to the company, more than 5,000 healthcare organizations in Europe use the product, with over 100 integrations into practice and hospital information systems. On the regulatory side, Tandem set the benchmark in 2026: in February its coding assistant, and in May the AI scribe itself, was certified under the EU MDR as a Class IIa medical device — for the scribe, according to the company, as the first vendor in Europe. This followed an inspection by the Swedish Medical Products Agency whose preliminary finding, according to Tandem's own summary, was: Class I is not enough for an AI scribe. This certification work deserves recognition; it raises the bar for the entire category. Tandem's German market presence has so far visibly focused on the outpatient sector.

Recare (Berlin)

Recare comes from digital discharge and transition management and, since a 37 million euro funding round led by DNV — since then its largest shareholder —, has positioned itself as an AI platform for hospitals, with modules for extraction, speech, and documents. According to the company, it works with over 1,000 hospitals. Its strength lies in the breadth of hospital relationships from the transition-management business. Anyone evaluating Recare should ask the same question as with every vendor on this list: how is it checked whether extracted content is supported by the source — and who checks it?

voize (Berlin)

voize specializes in voice documentation for nursing and is considered the market leader for it in the German-speaking region. In a Charité-led study, voize reduced nursing documentation time by 27 percent, according to the study. For facilities whose biggest pain point is nursing documentation, this is the most specialized solution on this list — with a deliberately narrow focus.

FICUS Health

FICUS Health addresses documentation in rehabilitation clinics and has built visibility there in a short time. The rehab sector, with its mix of DRV logic (DRV: German statutory pension insurance, which funds rehab), statutory health insurance requirements, and discharge reports, is a terrain of its own — that several specialized vendors now exist for it speaks to the size of the problem.

aiomics (Berlin)

We belong on this list with a different premise: aiomics is not a scribe but the verification layer on top of hospital IT. The system reads in unstructured documents — faxes, PDFs, referrals, questionnaires —, checks every extracted statement against the source document, and discloses for every field where it came from. On top of this verified record run admission and referral management as well as the Falldialog (the pre-review case dialogue with payers); a conversation documentation tool for the admission interview is also in production, deliberately narrow in scope. aiomics is live in use in German rehab clinics, ISO 27001 certified (TÜV Nord), processes exclusively in the EU, and its accuracy is being independently evaluated at Charité. MDR Class IIa certification for the conversation documentation is in preparation. To the best of our market knowledge, no other vendor on this list currently checks new data systematically against the existing patient record — that is exactly what we built aiomics for, and exactly what you should measure us against.

The comparison along the criteria that matter in operation

  • Modality: Tandem processes the spoken conversation; Recare documents and speech in a platform approach; voize nursing dictation; FICUS rehab documentation; aiomics existing documents plus the admission interview, with the verified record at the core.
  • Medical device status: Tandem is, according to the company, Class IIa certified for its scribe and coding assistant. aiomics is deliberately positioned outside the medical-device boundary where document and process work is concerned, and is preparing Class IIa certification for its conversation documentation. For all vendors: ask to see the qualification documentation, not just the outcome.
  • Data residency: Ask for the specific processing location and whether voice and document data leave the vendor's own hosting. aiomics runs its speech recognition self-hosted in the EU; audio is deleted by default after 72 hours with cryptographic proof.
  • Verification: The core question of the category. Generative systems make errors more fluent and therefore more credible. What matters is whether the system links every statement to its source — and whether unsupported statements are blocked or merely flagged.
  • Evidence: voize points to a Charité-led study; aiomics is being independently evaluated at Charité. Everything else on this list is vendor-reported — a difference worth keeping in mind when comparing.

Five questions to ask every vendor

  1. Does the system check its outputs against the source document and the existing record — and what happens to statements for which it finds no source?
  2. Where does processing run, who are the subprocessors, and what is verifiably deleted, and when?
  3. How does the system handle omissions? Studies show that missing information is more common in AI documentation than fabricated information — asking only about hallucinations tests the smaller error class.
  4. What action does the system require from physicians before content is adopted — a bulk confirmation or a traceable item-by-item sign-off?
  5. How is the medical device status justified and documented — regardless of what it turns out to be?
